Set Up a Memorial Gift

If you would like to honor your loved one with a Memorial Gift to Dragonfly Wings, fill out the form below and we will set up the online gift form, and send the link to the designated email address. We will also post a photo of your loved one on our Memorial page, the link to send a gift, and we will collect checks and/or cash left with the funeral home or a designated individual. We will also post the link on our FaceBook page. Unless designated for a specific program or project, we will gift the donation in your loved ones name, based on need, as determined by our board.

We will contact you within a few hours of submission to verify the details and the set up of your Memorial.
